Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social service agencies.
• Experience using EMR and able to document narratively.
• MS Office proficiency: Word, Outlook, Excel.

Responsibilities

• Assess and evaluate members' needs to guide toward appropriate resources.
• Conduct telephonic and in-person assessments to plan care.
• Develop and implement personalized care plans based on clinical judgment.
• Coordinate services across providers and community resources.
• Document incidents and report per policy; support transitions of care.
• Collaborate with interdisciplinary team to optimize care and outcomes.
