Job summary

Humana is seeking a Field Care Manager 2 (Physical Health) to assess and coordinate care for Medicaid members in Louisiana. You will guide members and families to appropriate resources, address barriers to care, and collaborate with providers to support optimal wellbeing.

This field-based role requires an active RN license in Louisiana, 2+ years of clinical experience with diverse populations, and strong documentation skills.

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years of clinical experience with diverse populations (youth, adults, geriatrics).
• Knowledge of community health and social service agencies.
• Experience with electronic medical records and narrative documentation.
• Proficient in Word, Outlook and Excel.
• Ability to work independently with sound judgment and adapt care plans.

Responsibilities

• Engage and empower members and families using holistic strategies to support recovery, health maintenance, and participation in wellness programs.
• Conduct telephonic and in-person assessments to guide members toward appropriate resources for optimal health outcomes.
• Continuously monitor member progress with regular assessments to advance toward individualized care goals.
• Develop and implement personalized care plans based on clinical judgment and psychosocial factors.
• Collaborate with healthcare providers and community organizations to facilitate quality, cost-effective care.
• Coordinate delivery of essential services addressing physical health and social determinants of health.
• Facilitate communication across the interdisciplinary care team for seamless care transitions.
• Align daily practices with departmental strategies and objectives.
• Document and submit incident reports per organizational policies.
