Job summary

Humana is seeking an RN Field Care Manager 2 (Physical Health) to assess member needs and guide families toward appropriate resources for optimal wellness. You will work within the Louisiana Medicaid Team, delivering holistic, member-focused care and coordinating with providers to ensure quality outcomes.

The role emphasizes independent clinical judgment, care plan adaptation, and strong collaboration with community resources, while supporting transitions of care and documentation compliance.

Qualifications

• Active, unrestricted RN license in Louisiana.
• ≥2 years of clinical experience with diverse populations.
• Knowledge of community health and social service agencies and resources.
• Experience using EMRs and providing narrative documentation.
• Proficiency in Word, Outlook, and Excel.

Responsibilities

• Engage and empower members and families using holistic strategies for recovery and wellness.
• Conduct telephonic and in-person assessments to guide members to resources for optimal health.
• Monitor progress with regular evaluations toward individualized care goals.
• Develop and implement personalized care plans based on health status and psychosocial factors.
• Collaborate with providers, organizations, and services to ensure quality, cost-effective care.
• Coordinate delivery of essential services addressing physical health and social determinants.
• Facilitate communication across the care team for seamless transitions of care.
• Document incident reports in line with policies and regulatory requirements.
