Humana is seeking a skilled RN, Field Care Manager 2 (Physical Health) in Louisiana. You will assess member needs, develop personalized care plans, and collaborate with providers to coordinate physical health and social services.

The role emphasizes patient-centered care, community engagement, and telephonic and in-person assessments. The position is primarily field-based with a remote home office when not visiting members, requiring up to 50% travel within the Louisiana regions to support care

Qualifications

• Active, unrestricted RN license in Louisiana
• Two+ years clinical experience with diverse populations
• Knowledge of community health and social service resources
• Experience with electronic medical records and narrative documentation
• Proficient in Word, Outlook, and Excel; strong keyboarding and web navigation skills
• Ability to work independently and adapt care plans as needed

Responsibilities

• Engage and empower members and families to support recovery and wellness programs
• Conduct telephonic and in-person assessments to guide to resources
• Monitor member progress and reassess to meet goals
• Develop and implement personalized care plans based on clinical judgment
• Collaborate with healthcare providers and community organizations to ensure quality, cost-effective care
• Coordinate delivery of services addressing physical health and social determinants of health
• Facilitate communication across interdisciplinary care team for seamless transitions
• Align daily practices with departmental strategies and objectives
• Document and submit incident reports per policy and regulations
