Qualifications

• Graduate of an accredited school of nursing.
• Current unrestricted RN license in Texas.
• 3+ years of chronic illness/work with disabilities in case management or similar role.
• 2+ years in community health settings.
• 2+ years in case management or direct care settings.
• 2+ years of experience with MS Word, Excel, Outlook.
• Proven communication and analytical skills.
• Ability to travel within the El Paso TX area.
• Bilingual English and Spanish.

Responsibilities

• Conduct holistic evaluations of member needs across health and social services.
• Educate members on options for service delivery models.
• Perform initial and follow-up assessments within contractual guidelines.
• Coordinate care with healthcare teams and family members.
• Develop individualized care plans and coordinate referrals.
• Track and document assessments, care plans, and referrals.
• Participate in interdisciplinary team meetings.
• Monitor services for timeliness and quality of care.
• Advocate for patient needs with care teams.
