Qualifications

• 2+ years using MS Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic or face-to-face holistic evaluations of members' needs and preferences.
• Provide education and support to members on service delivery models.
• Perform initial and follow-up assessments and outreach calls.
• Identify members for high-risk complications and coordinate care.
• Manage members with chronic illnesses and disabilities for efficient health benefit utilization.
• Assess, plan, and implement individualized care strategies.
• Utilize company and community resources for effective case management.
• Collaborate with members, families, and healthcare providers to develop care plans.
• Identify and initiate referrals for social service programs.

Education
