Job summary

UnitedHealthcare is seeking a Care Coordinator to apply clinical expertise in coordinating care for medically complex members. You will work directly with members to ensure access to services and providers, using six CMSA activities and evidence-based practices.

This field-based role allows remote work within commutable distance to Shelby County, TN, with travel as required. You will collaborate with Case Management, Utilization Review, and Behavioral Health teams to optimize care and outcomes.

Qualifications

• Current Nursing License (RN).
• 2+ years of clinical RN experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Excellent organization, communication and time management skills.
• Residency in or near Shelby County, TN area.
• Access to reliable transportation and valid US driver’s license with up to 75% travel within territory.

Responsibilities

• Apply clinical knowledge to coordinate case management for Medicare/Medicaid members.
• Incorporate evidence-based clinical practices into coordination activities.
• Deliver member-centered self-management support and education.
• Ensure appropriate utilization of health benefits and timely documentation.
• Conduct telephonic and face-to-face assessments per policy and timelines.
• Advocate for members and resolve barriers to care.
• Coordinate community resource referrals to members and providers.
