Job summary

Qualifications

  • Current nursing license (RN) or Licensed Social Worker.
  • 2+ years of clinical experience as an RN or Social Worker.
  • 1+ years of experience with MS Office (Word, Excel, Outlook).
  • Valid driver’s license and ability to travel up to 75% within assigned territory.
  • Reside in or within 50-mile radius commutable distance to Coffee, Franklin, Van Buren, Warren or Grundy Counties.

Responsibilities

  • Apply clinical knowledge to effective case management for Medicare/Medicaid benefits.
  • Incorporate evidence-based clinical practices into care coordination activities.
  • Deliver member-centered, individualized self-management support and education.
  • Install appropriate equipment or supplies and educate on safe use.
  • Facilitate group visit strategies as defined by site needs.
  • Ensure appropriate utilization and consistent application of health benefits.
  • Conduct telephonic and face-to-face assessments per policy and timeframes.
  • Serve as member advocate to resolve barriers to care.
  • Provide education and coordination of community resource referrals.
  • Develop and maintain member-centered care plans with collaboration.
