Job summary

Qualifications

• Current Nursing License (RN)
• 2+ years of clinical RN experience
• 1+ years of MS Office experience (Word, Excel, Outlook)
• Excellent organization and time management skills
• Reside in or within commutable distance to Shelby County Counties
• Access to reliable transportation and US driver's license with ability to travel up to 75% within assigned territory

Responsibilities

• Coordinate member care needs for medically complex individuals.
• Apply evidence-based clinical practices to care coordination.
• Provide member-centered self-management support and education.
• Ensure appropriate utilization and access to Medicare/Medicaid benefits.
• Conduct assessments and document in Health Plan systems.
• Collaborate with Case Management, Utilization Review, and Behavioral Health teams.
