Job summary

UnitedHealthcare is seeking a Care Coordinator to apply clinical knowledge to coordinate care for medically complex members, ensuring access to services across acute, home and chronic settings.

This field-based role may be remote within commutable distance to Shelby County, TN, with travel up to 75% within the territory. A valid RN license and 2+ years of clinical RN experience are required; BSN or Master's in Clinical Field preferred.

Qualifications

• Current Nursing License (RN) is required.
• 2+ years of clinical RN experience.
• Proficiency with MS Office tools (Word, Excel, Outlook).
• Excellent organization, communication and time management.

Responsibilities

• Coordinate case management for Medicare/Medicaid beneficiaries.
• Deliver education and self-management support to members.
• Document and monitor all aspects of member care coordination.
• Collaborate with Case Management, Utilization Review and Behavioral Health teams.
