Job summary

UnitedHealthcare is seeking a Care Coordinator to apply clinical knowledge in coordinating Medicare/Medicaid member care. This field-based role supports medically complex members with remote work options within driving distance to Shelby County.

Responsibilities include assessments, care planning, member education, and coordination with Case Management and Behavioral Health teams to ensure timely access to services. An RN license and 2+ years of clinical experience are required.

Qualifications

• Current Nursing License (RN) in the US.
• 2+ years of clinical RN experience.
• 1+ years of MS Office experience including Word, Excel, Outlook.
• Strong organization, communication and time management skills.
• Reside in or within commuting distance to Shelby County.
• Ability to travel up to 75% within assigned territory to meet with members and providers.

Responsibilities

• Coordinate member care using clinical knowledge for Medicare/Medicaid.
• Incorporate evidence-based clinical practices into care coordination.
• Deliver member-centered self-management support and education.
• Ensure appropriate utilization of health benefits.
• Conduct telephonic and face-to-face assessments per policy.
• Serve as member advocate to resolve barriers to care.
• Refer to community resources as needed.
• Develop and maintain member-centered care plans.
• Collaborate with Care Coordination team for quality and efficiency.
• Monitor hospital encounters and intervene as needed.
• Escalate decisions beyond authority to manager.
• Participate in Quality Improvement processes.
• Use plan software to document service delivery and requests.
• Maintain time-sensitive documentation per regulations.
• Coordinate services per policy.
• Perform other duties as assigned.
