Qualifications

• Current, unrestricted RN license.
• 2+ years of clinical experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).
• Willingness to travel up to 75% within territory.
• Residence in Mecklenburg County, NC or surrounding areas.
• Access to a dedicated quiet home workspace with PHI security.

Responsibilities

• Assess, plan and implement individualized care management interventions.
• Identify and initiate referrals for healthcare and community services.
• Develop and implement care plans as a single point of contact.
• Communicate health information to stakeholders for coordinated care.
