Qualifications

• Minimum 5 years of clinical case management experience or equivalent education and experience.
• Associates or Bachelor's degree in Nursing; BSN preferred.
• Current, unrestricted RN license in the applicable state.
• Ability to communicate effectively and collaborate with diverse groups.
• Strong problem-solving, organizational, and time management skills.

Responsibilities

• Ensures members have access to appropriate health services based on their needs
• Identifies and manages members according to established criteria, facilitating necessary authorizations and referrals
• Monitors and evaluates the effectiveness of care management plans, making modifications as needed
