Job summary

CVS Health is seeking a compassionate Case Manager RN to support members through personalized, high-quality care coordination in a fully remote role. You will use clinical expertise, data-driven insights, and advocacy to help members achieve optimal, cost-effective health outcomes.

Responsibilities include assessments, care plans, discharge planning support, and advocating for benefits. Requires 3+ years acute care RN experience and active RN license; remote work across states with full-time

Qualifications

• Active, unrestricted RN license in your state of residence.
• Willingness to obtain additional state licenses (company paid).
• 3+ years of acute care RN experience (Med-Surg or Critical Care preferred).
• Strong clinical judgment, decision-making, and organizational skills.

Responsibilities

• Conduct comprehensive clinical assessments and identify care coordination needs
• Develop, implement, and monitor individualized case management plans
• Support inpatient review and discharge planning as appropriate
• Advocate for members to maximize benefits and access appropriate services
• Engage members through coaching, education, and motivational interviewing
• Identify and elevate quality-of-care concerns
• Deliver exceptional customer experience while collaborating with care teams and Medical Directors
