Job summary

CVS Health is seeking a Case Manager RN to support members through personalized, high‑quality care coordination in a fully remote nationwide role. You will conduct assessments, build care plans, assist discharge planning, and advocate for access to services, collaborating with care teams and Medical Directors.

An active RN license and 3+ years in acute care are required; multi‑state licenses and CCM are preferred. This full‑time position offers meaningful work and competitive benefits.

Qualifications

• Active, unrestricted RN license in your state of residence.
• 3+ years of acute care RN experience (Med‑Surg or Critical Care preferred).
• Willingness to obtain additional state licenses (company paid).
• BSN preferred.

Responsibilities

• Conduct comprehensive clinical assessments and identify care coordination needs.
• Develop, implement, and monitor individualized case management plans.
• Support inpatient review and discharge planning as appropriate.
• Advocate for members to maximize benefits and access appropriate services.
• Engage members through coaching, education, and motivational interviewing.
