Responsibilities

• Provide telephone-based nursing support and triage using approved protocols.
• Support patients with chronic-care management and self-management.
• Assess acute healthcare needs and determine appropriate level of care.
• Connect patients with primary care, specialty providers, and community resources.
• Promote patient engagement, wellness, and effective healthcare navigation.
• Collaborate with an interdisciplinary care team to improve outcomes and reduce hospitalization risk.
• Answer general health questions and provide health education.
• Deliver compassionate, holistic, patient-centered support.
• Utilize EMR to document interactions and coordinate care.
