Job summary

Comunidade Metodista is seeking a qualified Registered Nurse (RN) to support patients in navigation, prevention, and management of health through Remote Patient Monitoring and Chronic Care Management. You will review benefits, educate on home monitoring devices, and create personalized care plans via phone or video visits.

The role emphasizes ongoing patient engagement, addressing barriers, coordinating with virtual team members, and serving as first contact for non-emergent needs.

Qualifications

• Active, unrestricted Georgia RN license (Compact preferred).
• At least 3 years of experience in adult health.
• Background in adult chronic health conditions (Medical Surgical, Outpatient/Clinic, Care Management).
• Variety of Electronic Medical Records (EMR) experience.
• Proficient in basic computer skills and problem solving.
• Excellent communication and teamwork skills.
• Comfortable working remotely but collaboratively.

Responsibilities

• Welcome patients into continuous care programs and review benefits.
• Educate patients on home monitoring device use and frequency.
• Create personalized care plans via phone or video visits.
• Identify and address barriers to patient success.
• Provide education and coaching on chronic conditions.
• Review readings and update care plans monthly.
• Serve as first contact for non-emergent needs (med refills, scheduling).
• Collaborate with virtual team and in-office staff to meet patient needs.
