CVS Health is seeking a Health Coach Consultant for the 24-Hour Nurse Line. The role uses a collaborative process of assessment, planning, implementation, and evaluation to educate and influence members toward optimal health.

Qualifications

• Active unrestricted RN license in a compact state.
• 3+ years of clinical experience in disease management, ER, CCU, medical/surgical, telehealth or oncology.
• Preferred: coaching experience and telehealth familiarity.

Responsibilities

• Assess, plan, implement and evaluate member health status.
• Educate and influence member decisions to achieve optimal health.
• Collaborate with care teams and document care using electronic tools.
