Qualifications

• Must have an active and unrestricted PA Registered Nurse license.
• 3 years of combined clinical, case management, or disease management experience required.
• Strong clinical knowledge and ability to communicate effectively.

Responsibilities

• Provide health coaching and case management services.
• Assist members in navigating healthcare systems and coordinate care.
• Develop and monitor care plans for individual members.
• Engage with members to support self-management and lifestyle improvements.
• Provide exceptional customer service and maintain accurate documentation.
