Job summary

ArchWell Health is seeking a Chronic Disease Management Nurse to provide care management for patients with complex chronic conditions, including social and mental health co-morbidities. The role partners with the care team to deliver patient-centered, culturally sensitive care and to meet performance standards.

You will assess diverse patient needs, develop care plans, educate patients, and coordinate with social workers and PCPs.

Qualifications

• Active RN license in the state of employment is required.
• Minimum 2 years’ clinical work experience.
• Minimum 2 years’ case management experience.
• Minimum 1 year disease management experience.
• CCMC/CMCN certification preferred or willingness to obtain.
• Knowledge of ICD-10, CPT, HCPCS is preferred.
• Experience with CMS/HEDIS guidelines preferred.

Responsibilities

• Assess patients' physical, functional, social, psychological, and financial needs.
• Create and implement care plans for complex chronic conditions.
• Monitor progress and reassess goals and health status.
• Provide intensive disease state education and motivational interviewing.
• Coordinate with social workers and community resources.
• Support safe discharge planning with hospital case managers.
• Travel up to 10–20% locally to ArchWell Health centers.
