Qualifications

• Active clinical license in RN, RD, or MH field.
• Experience in case management and care coordination preferred.
• Excellent communication and collaboration skills.

Responsibilities

• Engage members and caregivers to understand barriers to optimal health.
• Develop collaborative care plans to improve self-management and outcomes.
• Coordinate care across physical, behavioral health, and social needs.
• Conduct initial and ongoing assessments and deliver telephonic/digital engagement.
