Job summary

CVS Health Corporation is seeking a Case Manager RN for a 100% remote role. No travel is expected with this position. The shift is Monday–Friday, 8:00 a.m. to 5:30 p.m.

Arizona time. You will telephonically assess, plan, implement and coordinate case management activities for members of our Federal Plans. Requires active RN licensure, ability to license in non-compact states, 3+ years of clinical practice, and 1+ year MS Office experience. An Associate's degree is required, BSN preferred.

Qualifications

• Active, current RN licensure in state of residence and ability to be licensed in all non-compact states.
• Must be willing and able to work Monday through Friday, 8 hour shift between 8am to 5:30pm Arizona time.
• 3+ years of clinical practice experience required.
• 1+ years of experience utilizing MS Office suites.

Responsibilities

• Telephonically assess, plan, implement and coordinate all case management activities for members of our Federal Plans.
• Evaluate medical needs to promote member wellness and outcomes.
• Develop proactive course of action addressing issues presented by members.
