Job summary

CVS Health is seeking a qualified RN Case Manager to work remotely from anywhere in the United States. The role involves telephonic case management, evaluating benefits and member needs, and coordinating with clinical teams to improve health outcomes.

A university-level or diploma nursing education plus strong documentation skills are required. You will support members across multiple states, adhering to URAC standards, and you must hold an active RN license (NLC or state-specific).

Qualifications

• 5+ years of RN experience, including at least 1 year in a hospital setting.
• Active multistate license via the Nurse Licensure Compact (NLC) for practice across participating states; non-compact states require individual state licenses.
• 1+ years' experience documenting electronically using a keyboard.
• 1+ years' experience in Oncology.

Responsibilities

• Telephonic case management with patients and care teams for fully or self-insured clients.
• Apply criteria, clinical guidelines, and policies to determine benefits and member needs.
• Use clinical judgment to reduce risk factors and address barriers in care planning.
• Assess information from multiple sources to address co-morbidities affecting functionality.
• Collaborate with supervisors and teams; present cases at case conferences for multidisciplinary input.
• Coordinate with clinical colleagues and programs to overcome barriers to goals and objectives.
• Follow case management processes compliant with regulatory and company policies.
• Use motivational interviewing to maximize member engagement and assess health needs.
• Identify and escalate member needs following set guidelines and protocols.
• Proactively reach out to members to guide their care.
