Job summary

CVS Health is seeking a remote RN Case Manager to support members across multiple states. You will conduct telephonic assessments, apply clinical guidelines, and coordinate care with the member's care team.

The role requires 5+ years as an RN with case management experience, multistate licensure via NLC or state licenses, and strong communication. It is a full-time position with comprehensive benefits.

Qualifications

• 5+ years of RN experience, including hospital setting.
• RN Case Manager role across multiple states with NLC multistate license or state-specific licenses.
• 1+ years' experience documenting electronically.
• 1+ years' Oncology experience.

Responsibilities

• Telephonic case management with patients and care team for fully or self-insured clients.
• Apply criteria and clinical guidelines to assess benefits and member needs.
• Use clinical judgment to reduce risk factors and address complex health indicators.
• Assess conditions including co-morbidities affecting functionality and care planning.
• Consult with supervisors and multidisciplinary teams to meet goals in case conferences.
• Collaborate with clinical colleagues to overcome barriers to goals.
• Follow case management processes in line with policies and regulations.
• Use motivational interviewing to maximize member engagement and assess health needs.
• Identify and escalate member needs per guidelines; actively reach out to guide care.
• Perform medical necessity reviews.
