Job summary

CVS Health is seeking a Case Manager RN for a 100% remote role with no travel. Standard hours are Monday–Friday, 8:00 am–5:30 pm Arizona time.

You will telephonically assess, plan, implement and coordinate case management for Federal Plans, evaluate medical needs, promote wellness, and advocate for members to maximize health outcomes. Requirements include an active RN license, eligibility in non-compact states, 3+ years clinical practice, 1+ year MS Office experience; Associate's degree

Qualifications

Active, current RN licensure in state of residence and able to be licensed in non-compact states.
Mon-Fri 8-hour shift between 8am to 5:30pm Arizona time.
Must live in PST, MST, or Arizona Time zones.
3+ years of clinical practice experience.
1+ years of experience utilizing MS Office suites.

Responsibilities

Assess, plan, implement and coordinate case management via phone.
Evaluate member medical needs to promote wellness and outcomes.
Develop proactive actions addressing issues to improve short/long-term outcomes.
Advocate for patients to maximize health coverage.
Promote quality and cost-effective outcomes and improve program efficiency.
Identify and elevate quality of care issues through established channels.
Provide a very high level of customer service.
Assess health literacy, technology capabilities, and readiness to change.
Use motivational interviewing to engage members and promote behavior changes.
