Job summary

Premera Blue Cross seeks a licensed RN/ARNP with case management experience to work remotely as a Case Manager. The role focuses on whole-person care, coordinating with clinicians to assess barriers and create member-centered care plans. Training is provided and ongoing education is supported.

You will collaborate with clinicians to monitor progress, address social determinants of health, and promote health outcomes while working within Pacific Time for training and scheduling.

Qualifications

• Active WA State or Compact RN or ARNP license required; 3 years post-licensure direct care experience.
• 1 year of post-licensure case management experience involving care coordination, care planning, discharge planning, or management of patient care over time.
• CCM certification preferred; if not held at hire, must obtain within 24 months of hire.

Responsibilities

• Engage members who face medical utilization disorders and build rapport for open dialogue.
• Conduct initial holistic health assessments identifying social determinants of health (SDOH) and cultural influences.
• Collaborate with members, caregivers and providers to develop a member-centric case management plan with goals and barriers.
• Monitor and update the case management plan to ensure progress toward goals.
• Facilitate communication among the health care team to maximize appropriate care.
• Provide interventions to remove barriers and promote positive behavior change using motivational interviewing.
