Job summary

UnitedHealth Group is seeking a Hospital Safe Discharge Case Manager to coordinate proactive discharge planning across the acute care continuum. The role drives efficient discharges with internal and external stakeholders, leveraging EMR systems and care coordination best practices.

This remote position offers EST-based scheduling (8AM-5PM EST) and requires an RN with a BSN, 5+ years inpatient nursing experience and 2+ years discharge planning.

Qualifications

• Bachelor of Science in Nursing
• Current, unrestricted RN license - Compact State License
• 5+ years of nursing experience in an acute care setting
• 2+ years of discharge planning experience
• Knowledge of utilization management and quality improvement
• Computer proficiency in Microsoft Office applications including Outlook, Word, and Excel
• Ability to read, analyze and interpret information in medical records and health plan documents
• Proven ability to use critical thinking, nursing judgement, and decision-making

Responsibilities

• Assess barriers to a safe and efficient discharge, coordinate resources and drive discharge with internal and external stakeholders
• Demonstrates knowledge of acute inpatient case management, utilization management and care coordination
• Manage assigned case load efficiently using time management skills
• Prepare and lead rounds to discuss case needs and transitions of care
• Utilize EMR systems to document and communicate care coordination across teams
• Consult Medical Director or leadership for complex cases
• Collaborate with hospital staff, patients and families to promote safe discharge plans
