CVS Health’s Aetna team seeks an experienced Case Management Coordinator to advance patient care. You will support case management by scheduling appointments, guiding members to benefits, and using care management tools to educate and assist members through complex health and social care needs.

The role requires 3–5 years of clinical experience, strong analytical and communication skills, and the ability to work independently from a home-based setup with travel in the Kane/DuPage Counties area.

Qualifications

• Must reside in Illinois.
• Willing to travel 50–75% around Kane and DuPage Counties, IL.
• 3–5 years clinical practical experience.
• Ability to work from home and collaborate virtually.
• Strong analytical and problem-solving skills.
• Effective communication, organizational and interpersonal skills.
• Proficiency with MS Word, Excel, Outlook and PowerPoint.
• Experience with corporate software applications.
