CVS Health in Illinois seeks a Case Management Coordinator to support the Aetna team, coordinating care for members with Medicare/Medicaid in Kane and DuPage counties.

You will assist with scheduling, benefits education, and collaboration with providers, members, and community partners, while working from home and traveling up to 50-75%.

Requires RN license in Illinois, 3-5 years clinical experience, strong analytical and communication skills, and proficiency with MS Office.

• Willing to travel up to 50-75% from home in Kane & DuPage Counties, IL.
• Minimum 3-5 years clinical practical experience.
• Independent worker with virtual collaboration capabilities.
• Excellent analytical and problem‑solving skills.
• Strong communication, organizational and interpersonal skills.
• Proficient with MS Word, Excel, Outlook, PowerPoint and proprietary apps.
