Job summary

The position requires RN licensure in Illinois and 3–5 years of clinical experience. Strong analytical, communication, and independent-working abilities are essential.

Qualifications

• Must reside in the state of Illinois.
• Must possess reliable transportation and travel up to 50-75% around East Central Illinois; mileage reimbursed.
• Minimum 3-5 years clinical practical experience.
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently.
• Proficient with standard corporate software including MS Word, Excel, Outlook, and PowerPoint, plus some proprietary apps.
• 2-3 years Care Management, discharge planning or home health care coordination experience.
• Certified Case Manager.
• Bilingual.

Responsibilities
