Job summary

CVS Health is seeking a Case Management Coordinator to support Aetna members enrolled in Medicare and Medicaid, coordinating appointments and benefits education from a home-based role in the Chicago area.

The role requires Illinois RN licensure, an Associate’s degree (Bachelor’s preferred), 3–5 years clinical experience, strong analytical and communication skills, and the ability to work independently using secure systems.

Qualifications

• Minimum 3-5 years clinical practical experience.
• Must reside in the state of Illinois.
• Excellent analytical and problem-solving skills.
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently and navigate multiple systems.
• Proficient with MS Word, Excel, Outlook, and PowerPoint.

Responsibilities

• Coordinate case management to facilitate appropriate healthcare outcomes for members.
• Assist with appointment scheduling and benefits education for members.
• Use care management tools and resources to support members.
