The individual will assist with scheduling, benefit education, and access to care for members using care management tools to improve outcomes. Ideal candidates have 3–5 years of clinical experience, strong analytical and communication skills, and the ability to work independently in a home

Qualifications

• 3–5 years of clinical practical experience.
• Able to travel up to 75% from home location in and around West side/Chicago and Surrounding areas.
