CVS Health is seeking a Case Management Coordinator to join the Aetna team. The role focuses on coordinating care and assisting members in accessing benefits, with extensive travel across West Suburbs IL and surrounding areas while working from a home location.

Ideal candidates hold an Illinois RN license, have 3–5 years of clinical experience, and can collaborate virtually using multiple systems. Strong analytical and communication skills are essential for success in this hybrid, full-time

Qualifications

• Must reside in Illinois.
• Willing to travel up to 75% from home location in and around West Suburbs IL - including DuPage, Kane, DeKalb, Kendall and surrounding areas). Mileage is reimbursed per our company expense reimbursement policy
• Minimum 3-5 years clinical practical experience
• Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually
• Excellent analytical and problem-solving skills
• Effective communications, organizational, and interpersonal skills.
• Ability to work independently
• Effective computer skills including navigating multiple systems and keyboarding
• Demonstrates proficiency with standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint, as well as some special proprietary applications
