Job summary

Qualifications

• Must reside in Illinois.
• Willing to travel 50-75% within East Central Illinois (Champaign area).
• 3-5 years clinical practical experience.
• Independent work style with ability to collaborate virtually using multiple systems.
• Proficient in MS Word, Excel, Outlook, PowerPoint and some proprietary applications.

Responsibilities

• Coordinate case management activities and assist with appointment scheduling and follow-up.
• Help members access benefits and education through care management tools and resources.
• Collaborate with members, providers, and community organizations to address health care and social determinant needs.
• Communicate effectively to support care decisions and ensure quality outcomes.
