Job summary

CVS Health is recruiting a Case Manager RN for a 100% remote role, with hours Mon–Fri 8:00am–5:00pm in your residence time zone and occasional evenings, weekends, and holidays.

The ideal candidate will have an active RN license, 3+ years in acute care, and strong communication and collaboration skills to coordinate care and advocate for patients within benefit plans and health resources.

Qualifications

• Active and unrestricted RN license in state of residence or multi-state licensure.
• 3+ years of acute care experience as an RN.
• 3+ years of experience with Microsoft Office Suite.
• Strong problem-solving and clinical decision-making skills.
• Ability to manage sensitive information with professionalism and confidentiality.
• Excellent communication and relationship-building skills.
• Ability to work effectively with diverse patient populations and healthcare teams.

Responsibilities

• Apply data-driven methods to design tailored case management programs.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on plans and benefits.
• Advocate for patients to maximize health care coverage.
• Promote quality, cost-effective outcomes and program efficiency.
• Identify and escalate quality of care issues through established channels.
• Provide an expected very high level of customer service.
• Assess health literacy, technology capabilities, and readiness to change.
• Provide coaching, education, and support to empower ongoing self-management.
