Job summary

CVS Health is seeking a Case Manager RN for a fully remote role. The position supports members across states with flexible hours and a focus on coordinated care, discharge planning, and advocacy. You will assess needs, coordinate services, and work with care teams to optimize outcomes.

Qualified candidates hold an active RN license, at least 3 years in acute care, and strong communication skills. BSN is preferred; multi-state licensure may be required upon hire.

Qualifications

• Active, unrestricted RN license in the state of residence with ability to obtain multi-state licensure.
• At least 3+ years of acute care experience as an RN.
• Strong communication and problem-solving skills.

Responsibilities

• Apply data-driven methods to identify members for case management or referrals to programs.
• Conduct comprehensive clinical assessments and plan care accordingly.
• Evaluate needs and develop flexible approaches based on plans and benefits.
• Advocate for patients to maximize health coverage and outcomes.
• Promote quality, cost-effective results and suggest program improvements.
• Identify and escalate quality-of-care issues through established channels.
• Provide a very high level of customer service to members.
• Assess health literacy and technology readiness of members.
• Use motivational interviewing to engage members in healthy changes.
• Provide coaching, education, and support for independent medical decisions.
