Job summary

CVS Health is seeking a Case Manager, Registered Nurse (Remote) to work from home across the United States. The role involves independent nursing judgment, clinical assessments, discharge planning, and care coordination to optimize member health outcomes.

You will work Monday–Friday with some coverage evenings and weekends as needed, in a fully remote setup. A valid RN license and 3+ years acute care experience are required, with strong MS Office skills and HIPAA compliance.

Qualifications

• Active and unrestricted RN license in the state of residence with ability to obtain multi-state licensure upon hire.
• 3+ years of acute care experience as an RN.
• 3+ years of experience with Microsoft Office Suite.

Responsibilities

• Apply data-driven methods to identify members and tailor case management programs.
• Conduct comprehensive clinical assessments and develop flexible care plans.
• Advocate for patients to maximize health care coverage and services.
• Promote quality and cost-effective outcomes and suggest process improvements.
• Escalate quality of care issues through established channels and maintain HIPAA compliance.
