Job summary

CVS Health is seeking an RN Case Manager to join our remote team. You’ll assess, plan, implement, and coordinate care for members, using clinical judgment to promote wellness and appropriate resource use.

Candidates must live in EST and preferably hold a Compact RN license in Massachusetts. Normal hours are Monday through Friday, 8am-5pm in the residence time zone with occasional flex to meet member and business needs.

Qualifications

• Active RN license in state of residence, preferably compact
• RN living in EST Time Zone
• Willingness to obtain additional state licenses upon hire (company paid)
• 3+ years of acute care clinical RN experience (medical, post-surgical, ICU, pediatrics, discharge planning)
• 0-10% travel may be required

Responsibilities

• Assess, plan, implement, and coordinate case management activities with members to evaluate medical needs and wellness.
• Use clinical tools and data (assessments, claims, etc.) to evaluate needs and refer to plan sponsor programs.
• Apply clinical judgment to reduce risk factors and address complex health issues.
• Follow regulatory and company policies in case management processes.
• Use interviewing skills to engage members and assess health status and needs.
