Qualifications

• 2+ years case management, discharge planning or home health coordination.
• Dedicated private workspace and ability to work from home.

Responsibilities

• Coordinate case management for catastrophic and chronically ill members across the care continuum.
• Interact with members telephonically or in person; home visits as needed.
• Assess medical and vocational status; plan care to optimize wellness and return to work.
• Communicate with providers, employers, and insurers; document all case work activities.
• Provide education and prevention information; ensure regulatory and policy compliance.
