Job summary

CVS Health is seeking a dedicated RN to join our Community Care team. In this role, you’ll work with members, providers, and families to manage care across the continuum, coordinating case management activities and leveraging community resources to improve health outcomes.

You will assess medical and vocational needs, develop plans, and collaborate with an interdisciplinary team to deliver person-centered care.

Qualifications

• Active and unrestricted RN license (Compact License)
• 3+ years of clinical practice experience
• 2+ years case management, discharge planning, or home health coordination experience

Responsibilities

• Liaise with members, providers, insurers, and healthcare personnel.
• Coordinate all case management activities across the continuum of care.
• Interact telephonically or in person with members/clients.
• Assess medical and vocational status; develop care plans to optimize wellness.
• Document case work activities and communicate with stakeholders.
• Collaborate with internal multidisciplinary teams to optimize outcomes.
• Conduct outreach to physicians or specialists as needed.
• Provide education and prevention information for better outcomes.
