Job summary

Qualifications

• Florida residency is required.
• Active and unrestricted Florida RN license.
• 3+ years clinical experience with Medicare populations (diabetes, CHF, CKD, post-acute, hospice, palliative).
• 2+ years case management, discharge planning or home health coordination.

Responsibilities

• Act as liaison among members, providers, employers and insurers.
• Coordinate all case management activities for complex cases across care continuum.
• Assess member needs and develop care plans to optimize outcomes and return to baseline.
