Job summary

Northern Light Health, in strategic partnership with Optum, is seeking a Care Manager, RN to lead coordination of patient-centered care across the continuum. You will develop safe discharge plans, collaborate with patients, caregivers, and the multidisciplinary team, and optimize transitions in care to reduce readmissions.

The role offers the flexibility to telecommute from anywhere in the United States and requires strong leadership in care coordination, utilization management, and risk

Responsibilities

• Communicates directly with staff, physicians, patients, and families
• Coordinates patient care processes to achieve quality outcomes and optimize resource utilization
• Promotes continuity of care through patient and family education
• Reviews the patient plan of care with the multi-disciplinary team and coordinates conferences for complex cases
• Facilitates and participates in care conferences to coordinate interventions and continuity of care
• Implements nursing interventions with the multi-disciplinary team plan to enhance staff performance and patient care
• Assists in arranging schedules for physician rounds, team meetings and related opportunities to improve communication
• Adheres to name badge and dress code requirements
• Interfaces with Risk Management and staff to identify QA or risk issues and conducts follow-up as required
• Ensures compliance with CMS and Joint Commission Standards and regulatory requirements
