Job summary

Alignment Health is seeking a Remote RN Case Manager SNP to coordinate care for high-need seniors across states. The position requires an active California RN license (non-compact) and at least two years of clinical and one year of case management experience.

You will assess health status, facilitate referrals, educate patients, and collaborate with PCPs and the care team. Must be available 8:00am–5:00pm Pacific Time.

Qualifications

• Active RN license in California (Non-Compact).
• 1 year case management experience minimum.
• 2+ years clinical experience.
• Health Plan experience preferred.

Responsibilities

• Coordinates care for member, family, and physician.
• Facilitates referrals/authorizations within benefit structure.
• Completes comprehensive assessments within scope of practice.
• Collaborates with PCP and care team to implement plan of care.
• Interfaces with physicians and specialists on care plans.
• Provides education and self-management support.
• Assists in problem-solving with providers and claims.
• Maintains confidentiality of PHI per policy.
