Sagility seeks an experienced Senior Care Specialist (RN) to conduct telephonic health assessments for members and develop individualized care plans. This remote, seasonal role emphasizes nursing judgment and education while coordinating transitions of care and supporting beneficiaries with Medicare, Medicaid, or employer plans.

You will collaborate with a multidisciplinary team, document outcomes, and empower members toward better chronic condition management in a flexible Work From Home

Formación

• Active RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• 2+ years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical thinking, and problem solving.
• Ability to work independently in a remote environment.
• Proficiency in Microsoft Word, Excel, and Outlook.

Responsabilidades

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate benefits, services, and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Coordinate care among members, caregivers, providers, and care team.
• Document interactions, assessments, and outcomes accurately in systems.
