Job summary

Sagility is hiring a Senior Care Specialist (Registered Nurse) for a work-from-home position. The role involves telephonic health assessments, care planning, and patient education with a focus on Medicare, Medicaid, and group health plans.

The RN will coordinate care, support transitions, and work within multidisciplinary teams to optimize member outcomes. Seasonal assignment with a June 2027 tentative end date.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Compact license permitting practice in applicable states.
• Minimum of 2 years clinical nursing experience; 3+ preferred.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
• Help members navigate healthcare benefits and care options.
• Assess factors affecting health and well-being, including environmental and psychosocial aspects.
• Collaborate with multidisciplinary teams to support member care.
• Promote self-management through education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions, assessments, and outcomes accurately in systems.
