Sagility is seeking a Senior Care Specialist (Registered Nurse) for a seasonal, work-from-home role. You will conduct telephonic health assessments, develop individualized care plans, and build health coaching relationships to support chronic condition management.

Qualified RNs will have active licenses (state or compact), 2+ years nursing experience, and strong communication skills. The role emphasizes remote nursing judgment, ENR navigation, and coordination with a multidisciplinary team.

Formación

• Active, unrestricted RN license in the state of residence or compact license.
• 2+ years clinical nursing experience; 3+ preferred.
• Ability to work independently in a remote environment.
• Strong verbal and written communication skills.
• Proficient in EMR and MS Office tools.

Responsabilidades

• Conduct telephonic health assessments for members in Medicare/Medicaid or group health plans.
• Develop actionable, individualized care plans based on health risks.
• Establish health coaching relationships and provide education and support for chronic conditions.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning including DME and home health services.
• Navigate benefits and options for members and caregivers.
• Coordinate care among members, caregivers, and care team.
• Document interactions and outcomes accurately in systems.
