Job summary

Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) to perform telephonic health assessments and develop individualized care plans from a work-from-home setup.

The role emphasizes clinical judgment, patient education, and coordination of transitions of care for Medicare/Medicaid populations. A valid RN license and compact licensure are required, with remote work capability and strong communication skills being essential.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.
• Ability to efficiently navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being, including psychosocial elements.
• Collaborate with multidisciplinary teams to support member care.
• Promote self-management with education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions, assessments, and outcomes accurately in systems.
