Job summary

ArchWell Health in Charlotte, NC is seeking a Care Management Specialist to support Nurse Care Managers and Chronic Disease Managers in coordinating high-quality care for seniors. The role involves patient outreach, assistance with non-clinical needs, obtaining medical records, and ensuring smooth transitions.

You will document interventions in the EHR, collaborate with center teams, and provide administrative support to the care management team.

Qualifications

• High school diploma or equivalent required.
• MA, LPN, or CNA certification preferred.
• Minimum 5 years experience as MA/LPN/CNA, preferably in care management or acute care/facility.
• Proficient PC skills.
• Excellent customer service and patient-centric problem solving.

Responsibilities

• Conducts outreach to patients to address non-clinical needs to coordinate care, provide support, and connect with internal resources.
• Coordinates services such as follow up visits, confirming DME deliveries, ensuring home health care services are initiated, and coordinating transportation for medical appointments.
• Works collaboratively with Nurse Care Managers, Chronic Disease Managers, healthcare providers, and center teams to provide patient support.
• Maintains accurate and detailed documentation on outreach, interventions, and outcomes including communication with patients, nurse care managers, healthcare providers, and clinic teams.
• Works collaboratively with the Care Management department to ensure progress toward departmental and organizational goals.
• Provides administrative support to the Nurse Care Manager and Chronic Disease Manager.
