Responsibilities

• Develops, assesses, and facilitates complex care management activities for members with primarily physical needs.
• Creates ongoing care plans and coordinates with providers, specialists, and community resources.
• Identifies barriers to care and provides interventions and support to ensure person-centered care.
• Coordinates between members, families, and care teams to ensure timely access to services.
• Provides education to members and families on disease processes, care options, and benefits.
• Maintains member information for regulatory compliance.
• Performs outreach via telephonic or digital means to assess needs and progress.
• Supports leadership with opportunities to improve care delivery in a cost-effective manner.
