Qualifications

• Requires RN licensure in WA state or compact state.
• Experience with maternal/infant populations and behavioral health crises preferred.

Responsibilities

• Evaluates member needs and barriers to care; recommends and facilitates an appropriate care plan.
• Develops and updates care/service plans; coordinates with providers and community resources.
• Identifies problems/barriers to care and delivers care management interventions.
• Coordinates between member/family and care team to ensure person-centered care.
• Provides ongoing follow-up and monitoring; revises plans to meet goals.
• Provides resource support for local services (housing, employment, etc.).
• Facilitates timely access to needed care or services with providers.
• Conducts telephonic, digital, home, or other outreach as needed.
• Collects, documents, and maintains member information to comply with regulations.
• Educates members and families on disease processes, care options, and benefits.
• Provides leadership feedback on care delivery improvements.
• Other duties as assigned; complies with policies and standards.
